Download the Acute Hepatic Porphyria Emergency Department Wallet Card fillable PDF. Plan to save the file in a location that provides easy access.

Example: save to desktop.

1. Once downloaded, right-click on the file, select Open With and then select your

2. Card 1 is for information purposes only and is not available for editing.

3. On Card 2, you will fill in your information.

Click inside the empty box beside My Name is: and type your first and last name. Repeat

these steps for the following:

• Phone:

• Hematologist/MRP:

• Phone:

• Email:

Print your cards or save them to print later.

Cut out your card, leaving the middle border intact between cards one and two. Fold along the

middle line so that they are back-to-back.

Once folded, laminate your card. If you don't have a home lamination machine, visit your

nearest office supply store that offers in-store lamination services.

Tip: After lamination, round the corners to smooth the points of your AHP ED Wallet Card.
